How To Apply For Florida State Id Card
Driver License Required Documents
Federal and State law requires proof of identity, Social Security number, and two (2) proofs of residential address for a Florida driver license or ID card. Click here for a handy checklist of required documents.

1. Identification (bring 1)
U.S. Citizens:
- Original or certified U.S. birth certificate from a government agency
- Valid, unexpired U.S. passport or U.S passport card
- Naturalization certificate or certificate of citizenship
- Consular proof of birth abroad

Non-U.S. Citizens:
- Current permanent resident card (I-551)
- Current employment authorization card (I-688B, I-766)
- Valid passport along with proof of current temporary status classification from the Department of Homeland Security
Canadian Citizens:
- Canadian certificate of citizenship
- Canadian naturalization certificate
- Valid Canadian passport
- Original or certified Canadian birth certificate
2. Proof of ALL Legal Name Changes (if any)
Ever changed your name?
If the name on your identification document doesn't match your current legal name, you will need to show proof of all legal name changes to prove your current name.
- U.S. passport in current name – OR-
- Original or certified copies of ALL marriage certificates or court orders, such as divorce decrees, that show your name change(s). Photocopies and non-certified copies are not valid.

3. Social Security number (bring 1)
Note: This must show your full Social Security number and full name.
- Social Security card
- W-2 form (hand-written not acceptable)
- Paycheck stub showing applicant's name and full number (hand-written not acceptable)
- Internal Revenue Service form 1099
- U.S. Military ID card showing Social Security number
- Self-Certification of Social Security Number Form

Never been issued a Social Security number?
U.S. citizens and permanent residents must present an affidavit from the Social Security Administration verifying that they have never been issued a social security number. In addition, U.S. citizens, permanent residents and Canadian citizens without a social security number, as well as those with temporary status classifications from DHS, and must bring at least one proof of secondary ID. 4. Residential Address (bring 2)
Note: Each proof of address must show your complete residential address. P.O. boxes are not accepted.
- Deed, mortgage, monthly mortgage statement or payment booklet or rental agreement
- Florida voter registration card
- Florida vehicle registration or title
- W-2 form or 1099
- Utility bill, hook up or work order
- Automobile or homeowner's insurance policy or bill
- Mail from financial institutions, including checking, savings, or investment account statements
- Medical or health card with address
- Letter from shelter or half-way house verifying applicant lives at this address
- Educational transcripts
- Certification of Address form

No proof of residential address?
UPDATED AS OF JUNE 10, 2020:
Residing with a Relative
If you reside with a relative, you must provide two acceptable proofs of residential address documents in the relative's name, document(s) showing the family relationship, along with a Certification of Address form to self-certify.
Acceptable family relationship documents may include, but are not limited to:
- Marriage certificate
- Birth certificate
- Military dependent identification card
- Adoption record
If you are unable to provide family relationship documents, a completed Certification of Address form must be submitted. The person you live with who is signing the form can choose to sign the document in our office at the time of the transaction or sign the form in front of a notary.

Residing with a Non-Relative
If you reside with someone that is not a relative, you must provide two acceptable proofs of residential address documents in the non-relative's name along with a Certification of Address form to self-certify. The person you live with who is signing the form can choose to sign the document in our office at the time of the transaction or sign the form in front of a notary.
